Indications/Uses
Zithrocare 200 Non-gonococcal urethritis & cervicitis due to Chlamydia trachomatis. Taken alone or in combination w/ rifabutin for the prevention of disseminated Mycobacterium avium complex (MAC) disease in patient w/ advanced HIV infection. In combination w/ ethambutol for the treatment of disseminated MAC infection in patient w/ advanced HIV infection. Zithrocare 500 Resp tract (including otitis media), skin & soft tissue, & uncomplicated genital infections. Prophylaxis of MAC infections.
Dosage/Direction for Use
Adult Non-gonococcal urethritis & cervicitis caused by Chlamydia trachomatis in STD 1,000 mg as a single dose. Prophylaxis against MAC infections in patient infected w/ HIV 1,200 mg once wkly. Childn Total dose of 30 mg/kg given as a single daily dose of 10 mg/kg daily for 3 days or given over 5 days w/ a single daily dose of 10 mg/kg on day 1, then 5 mg/kg on days 2-5. Max total dose: 1,500 mg. Zithrocare 500 Adult Lower resp tract infection 500 mg once daily for 3 days or as an alternative 500 mg on day 1 followed by 250 mg once daily on days 2-5. URTI 500 mg once daily for 3 days. Community-acquired pneumonia 500 mg once daily for 3 days or as an alternative 500 mg bid on day 1 followed by 500 mg once daily for 4 days. Skin infections 500 mg as single dose on day 1 followed by 250 mg once daily for 4 days. Childn Acute otitis media 30 mg/kg as a single dose may be given as an alternative. Streptococcal pharyngitis Single dose of 10 or 20 mg/kg for 3 days. Max: 500 mg daily.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to azithromycin or any macrolide antibiotics. Co-administration w/ ergot derivatives.
Special Precautions
Severe renal impairment (GFR <10 mL/min). Pregnancy & lactation. Zithrocare 200 Rarely, serious allergic reactions including angioedema, anaphylaxis & dermatologic reactions including SJS & TEN. QT interval prolongation may lead to Torsade de Pointes & cardiac arrhythmia. Hepatotoxicity; Clostridium difficile associated diarrhea; exacerbation of myasthenia gravis; sexually transmitted infection; development of drug resistant bacteria. Zithrocare 500 Rarely, severe hypersensitivity reactions may be prolonged. Transient reduction in neutrophil count.
Adverse Reactions
Headache. Zithrocare 200 Angioedema & cholestatic jaundice, nausea, vomiting, diarrhea or abdominal pain; palpitations & chest pain; dyspepsia, flatulence, & melena; monilia, vaginitis & nephritis; dizziness, vertigo & somnolence; fatigue; rash, photosensitivity & angioedema. Zithrocare 500 GI disturbances.
Drug Interactions
Zithrocare 200 Increased serum conc w/ nelfinavir. May potentiate effects of oral anticoagulants eg, warfarin. Zithrocare 500 Carbamazepine, corticosteroids, cyclosporine, digoxin, ergot alkaloids, theophylline & warfarin. Antacid prep.
